Civil Assistant Surgeons in A.P. Insurance Medical Service
(General Recruitment)

It is hereby informed that on the basis of the merit in the Written Examination held on 05/08/2012 for Recruitment to the posts of Civil Assistant Surgeons in A.P. Insurance Medical Service (General Recruitment) vide Commission’s Notification No.42/2011, the candidates whose Register Numbers are given below have been called for Provisional Verification of Original Certificates in 2nd Spell on 1st & 2nd of Feb 2013 at 11:00 AM in the Commission’s Office subject to fulfilling the
conditions regarding the eligibility and other aspects to the recruitment. The Candidates are required to download the Call Letter (Memo), Check lists (Two), Attestation Forms (Two) & other relevant forms and also to produce the Original Certificates as mentioned in the official notification with one set of Photostat copies along with  two passport size Photos duly Attested by a Gazetted Officer. No separate communication will be sent in this regard.

• Date of Birth (SSC or its equivalent) in support of Minimum / Maximum age as on 01/07/2011.

• Proof of age relaxation in case of A.P State Govt. servant ( Regular Service Certificate from the concerned Dept. , Govt. of ANDHRA PRADESH/ NCC instructor Certificate, retrenched Census Service Certificate or any other certificate in respect of age relaxation claimed

• Integrated Community Certificate (issued by the concerned Tahsildar/M.R.O./R.D.O).

• Creamy Layer exemption certificate from Revenue authorities for BC candidates only, as per G.O.Ms.No.3, BC Welfare (C2) Dept., dt. 04/04/2006 who claim reservation under BC Community

• Study Certificates from IV class to X class if studied in school or Residence certificate for 7 years prior to appearing in SSC, if appeared for SSC as a private candidate.

• Original Qualification Certificates
  (a) M.B.B.S. Degree or an equivalent qualification as entered in the schedule to
  the Indian Medical Council Act, 1956 as subsequently amended.
  (b) Must be a permanent Registered Medical Practitioner within the meaning of
  the Law for the time being existing in the State.

• Hall ticket for the written examination held on 05/08/2012. in original.

• Check list – 2 sets.

• Two Attestation forms with photos duly filled in by the candidate and attested by a Gazetted Officer with Photos.

• 2 Latest Passport Size Photos.

• If, no further verification it is found that any Candidate is not eligible with reference to the particulars furnished by him/ her in the Application Form/ Certificate, the Candidature of such candidate/ Candidates shell be cancelled at any stage in the process of Selection.
